The Hook. D-PLEX₁₀₀ is doxycycline hyperconcentrated inside a polymer-lipid matrix that you sprinkle into an open abdominal wound at closure; it erodes layer by layer over 30 days, holding local antibiotic concentrations at the incision long after systemic prophylaxis has washed out and the surgical wound has cut itself off from the bloodstream.
The Bull Case. The FDA accepted the NDA on July 29, 2026 with Priority Review, no filing review issues, and a PDUFA target action date of November 28, 2026 — a full quarter ahead of the company’s own Q1 2027 guidance. The efficacy signal replicated: a 51% reduction in SSI rate in the SHIELD I >20 cm subgroup (8.5% → 4.2%, n=423) became a 60% reduction in the prospectively powered SHIELD II confirmatory trial (9.5% → 3.8%, p=0.0013), with near-identical control-arm rates. Azurity now commercializes in the U.S. and Canada; PolyPid keeps manufacturing worldwide, the platform, and everything outside North America. If D-PLEX₁₀₀ becomes standard adjunct care across open abdominal surgery and then cardiac sternal, this is a $300–500M global product against a fully diluted equity value near $140M today.
The Bear Case. Strip the headlines. Azurity — a specialty pharma that buys and commercializes marketed assets for a living — committed $30 million in near-term cash for U.S. and Canadian rights to a Phase 3-positive, Priority-Review NDA. Advanz committed €2.5 million for the EEA and U.K. in 2022. Both deals carry large back-end milestone stacks (up to ~$300M and up to €110M respectively), so the combined headline value is over $440M — but the combined cash two informed industry counterparties were willing to put on the table before approval is under $33 million, and the Advanz payment has sat in deferred revenue for four years without a single milestone earned. Meanwhile the FY2025 audit carries a going-concern explanatory paragraph, cash was $10.9M at March 31 against a ~$6M quarterly burn, the entire company (77 of 78 employees) and its single manufacturing facility sit in Israel, issued patents begin expiring in 2029, and 7.3 million warrants struck at $4.50 sit 6% out of the money.
